Summary:Sanford is responsible for ensuring qualified personnel
provide on-site diagnostic medical physics services under the terms
and conditions stated herein and must adhere to VHA Directive
1105.04 Fluoroscopy Safety, VHA Directive1105.05 Magnetic Resonance
(MR) Safety, and VHA Directive 1129 Radiation Protection for
Machine Sources ofIonizing Radiation. Medical devices that are used
to measure radiation or radioactive materials must comply
withNuclear Regulatory Commission (NRC) regulations and VHA permit
conditions for the facility under the VHA MML(Master Materials
License). These devices are required to be calibrated and tested
annually by qualified medical physicists to ensure the devices are
safe for measuring patient radiation doses, radiation being emitted
from patients, or radioactive samples.Sanford is seeking to procure
the services of a qualified Radiation Safety Officer (RSO)/
Diagnostic MedicalPhysicist(DMP) with emergency on-site visit
capabilities. Radioactive material use is limited to the Battle
Creek VAMedical Center (BCVAMC) for diagnostic medical uses under
the Nuclear Regulatory Commission (NRC) regulations 10 CFR 35.100
and 35.200 uses only. The contractor shall assist to ensure that
BCVAMC and WyomingCommunity Based Outpatient Clinic (WYCBOC) meet
the standards and regulatory requirements for radioactive material
use, diagnostic machine sources of ionizing radiation, and MRI,
consistent with:

OctoberHOURS OF WORK:1. Normal business hours vary by department or
service but are mainly 0800 to 1630. Service shall be coordinated
with the Sanford Representative to minimize the impact to patient
care.QUALIFICATIONS: i. Diagnostic Medical Physicist - All work
shall be performed by a qualified diagnostic medical physicist a.
Holds board certification in diagnostic radiologic physics or
radiologic physics by the American Board ofRadiology or in
Diagnostic Imaging Physics by the American Board of Medical
Physics, or in DiagnosticRadiological Physics by the Canadian
College of Physicists in Medicine b. Document the three (3) years
of experience for the modality being inspected c. Meet the ACR
Practice Parameter for Continuing Medical Education (ACR Resolution
17, 1996 - revised in 2017, Resolution 11). d. All work shall be
performed or supervised by a board-certified medical physicist who
meets the qualifications of a "Qualified Medical Physicist (QMP)"
as defined by the American Association of Physics inMedicine (AAPM)
for diagnostic medical physics. e. From AAPM, a Qualified Medical
Physicist meets each of the following credentials: f. Has earned a
master's and/or doctoral degree in physics, medical physics,
biophysics, radiological physics, medical health physics, or
equivalent disciplines from an accredited college or university;
and g. Has been granted certification in the specific subfield(s)
of medical physics with its associated medical health physics
aspects by an appropriate national certifying body and abides by
the certifying body's requirements for continuing education. ii.
Any of the following certifying bodies have been deemed appropriate
for diagnostic medical physics: a. The American Board of Radiology
b. The American Board of Medical Physics (currently ABMP doesn't
offer Diagnostic Medical Physics certification but already ABMP
certified Diagnostic Medical Physicists can maintain certification)
c. The Canadian College of Physicists in Medicine d. The American
Board of Health Physics including a minimum of three years relevant
experience in the subfield of medical health physicsSERVICES TO BE
PROVIDED1. Equipment Testing a. Contractor shall conduct equipment
inspections or quality control surveys on-site of the imaging
equipment listed below. b. Ensure the imaging equipment's
compliance with applicable Federal regulations and ACR
recommendations, and shall include, but not be limited to,
monitoring the following basic performance characteristics.2.
Diagnostic Medical Imaging including MRI: a. Performs all testing
and ensures policies and procedures are in accordance with current
guidelines established by JointCommission Accreditation, ACR,
including VA policy and procedures for all machine sources of
ionizing radiation including CT. This will need to be completed on
site.b. For MRI services, performs all testing, and ensures
policies and procedures are in accordance with current guidelines
established by Joint Commission Accreditation, ACR, including VA
policy and procedures annually. This will need to be completed on
site. c. Consults with practitioners and allied health personnel to
address the balance between radiation dose and image quality for
patient care when necessary. d. Develops specifications for imaging
equipment and diagnostic radiation detectors. e. Develops
procedures for the initial and continuing evaluation of imaging
equipment and diagnostic radiation detectors f. Provides evidence
of compliance of imaging equipment with specified regulatory and
accreditation agency rules and recommendations by submitting a
report of the findings to the COR (contracting officer's
representative). g. Applies other medical applications of physics
as appropriate to safely carry out diagnostic radiologic
procedures. h. Develops and applies Medical Health Physics
procedures associated with the practice of Diagnostic Radiology.3.
Diagnostic Use of Radioactive Materials in Nuclear Medicine:a. ACR
level Performance Testing on-site for two (2) Nuclear Medicine
cameras located at BCVAMC. b. Conducts Nuclear Medicine Service
audits on site and prepares written reports within 30 days on no
less than a quarterly basis. c. Ensures compliance with VA and NRC
requirements for the BCVAMC MML permit including record keeping
requirements. The contractor shall maintain a backup copy of their
work separate from records maintained at the medical centers. d.
Develops and manages policies and procedures for the inventory,
receipt, opening, and surveys of packages containing radioactive
materials. e. Ensures shipment of any radioactive materials is
performed consistent with regulatory requirements. This may require
DMP to be on site. f. Performs periodic surveys and wipe tests on
site as needed to ensure dose limits specified in 10 CFR 20 are not
exceeded, and that worker and public doses are maintained
consistent with as low as reasonably achievable (ALARA) principles.
g. Performs calibrations and quality control checks of radiation
survey and other radiation safety instruments on-site. h. Develops
and manages policies and procedures to ensure the security of all
radioactive materials is consistent with regulatory requirements.
This will require on-site visits. i. Completes uptake probe/well
counter resolution, reproducibility, constancy, and minimum
detectable activity(MDA) tests. This will require on site visits.
j. Ensures radiation survey meter calibrations, radionuclide sealed
source inventory and leak tests. This will require on- site visits.
k. Prepares license renewals and amendments. l. Completes fetal and
patient radiation dose estimate calculations as needed. m. Reviews
incidents involving radioactive material with respect to cause and
subsequent actions taken and prepare incident reports, and
preparation of medical event reports to the NHPP and/or NRC as
needed. This could require an on-site visit. n. Provides evidence
of compliance of nuclear imaging and radioactivity measurement
equipment with regulatory, professional and accreditation agency
rules and recommendations. o. Evaluates nuclear imaging and
radioactivity measurement procedures prior to clinical use. This
will require on-site visits. p. Develops and/or evaluates policies
and procedures related to the appropriate clinical use of radiation
for nuclear imaging, radiopharmaceutical therapy and/or
radioactivity measurement purposes. q. Completes review of
radiopharmaceutical dosimetry information noted in patient records.
This requires on-site visits. r. Develop and manage of a
comprehensive Quality Management Program that monitors, evaluates,
and optimizes nuclear imaging and radioactivity measurement
processes.s. Develops, maintains, and evaluates a comprehensive
clinical radiation safety program in nuclear medicine. t. Provides
consultation on patient or personnel radiation dose and associated
risks. u. Provides institutional consultation on program
development in medical nuclear imaging. v. Provide s safety
training for medical practitioners and other health-care providers.
w. Applies other medical applications of physics as appropriate to
safely carry out nuclear medicine procedures. x. Is reachable in
person or by telephone for emergency situations and or inspections.
y. Ensures dose calibrator accuracy, linearity, geometry, and daily
quality control tests.4. Serves as the institution's RSO5. Serves
as a member of the institution's Radiation Safety Committee (RSC)6.
Serves as a member of the institution's MRI Safety Committee,
performs MRI safety audits and training, finalizes decisions
regarding overall MRI Safety when necessary.7. Perform
comprehensive quarterly audits of the radiation safety program
including MRI and provide written reports to the RSC8. Perform an
annual review of the radiation safety program consistent with NRC
requirements and provide a written report to the RSC and brief
executive management of the results9. Provide consultation and
instruction regarding radiation safety to patients, staff, and for
public radiation safety10. Plan and specify thickness, material,
and placement of shielding needed to protect patients, workers, the
public and the environment from radiation produced incident to
diagnosis or treatment of humans11. Assess and evaluate installed
shielding designed to protect patients, workers, and the public
from radiation produced incident to diagnosis or treatment of
humans: a. Develop and monitor radiation protection equipment
specifications b. Develop and monitor procedures for the initial
and continuing evaluation of radiation protection equipment and
procedures c. Monitor compliance with radiation protection,
policies and procedures, regulations, accreditation organization(s)
standards, and national recommendations d. May perform acceptance
testing and commissioning of radiation protection equipment and
devices and commissioning of facilities e. May perform acceptance
testing and evaluation of radiation protection computer systems,
their algorithms, data, and output f. Evaluate radiation safety
procedures prior to use g. Development, management and evaluate of
the radiation safety program h. Provide radiation-protection
training for medical practitioners, and other health-care providers
i. Determine presence, evaluate, and assess any radiological hazard
resulting from the use of ionizing radiation or radioactivity for
compliance with appropriate regulatory and accreditation agencies
j. Shall always remain up to date any change requirements necessary
as mandated by all regulatory agencies k. Shall return all urgent
phone calls within two (2) hours and non-emergent phone calls
within twenty-four (24) hours.Is also able to report, on-site, to
respond to incidents/emergencies within four (4) hours when the
BCVAMC orWYCBOC deems the physicist's presence is essential l.
Review the results of lead garment testing annually m. Reviews
diagnostic imaging patient dosimetry information n. Develops and/or
evaluate of policies and procedures related to the appropriate
clinical use of radiation for imaging purposes including CT dose
optimization protocols. o. Develops and manage of a comprehensive
Quality Management Program that monitors, evaluates, and optimizes
imaging processes and delivered patient doses. p. Consults in the
development and/or evaluation of a comprehensive clinical radiation
safety program in diagnostic imaging. q. Consults and reviews
patient or personnel radiation dose and associated risks. r.
Provides diagnostic imaging physics training for medical
practitioners and other health-care providers.s. Provides
consultation to assure an optimized balance between image quality
and patient dose. t. Provides institutional consultation on program
development in diagnostic imaging. u. Reviews all employee
radiation dose reports and makes recommendation when needed. v.
Required on site approximately one (1) day per month for up to
eight (8) hours. w. May be required to be present onsite within
four (4) hours in the event of a radioactive spill. x. When NRC or
NHPP audit/visits occur the RSO may need to be present on-site per
