STARTING PAY: $21.01 - $30.46

Do you have---


A Bachelor's degree in a human services field (or advanced degree for higher pay)
Good organizational, verbal/written communication, computer, analytical and problem-solving skills.
Specialized training or experience in treating or working with a person who lives with a Mental Health Diagnosis (QMHP) and/or Intellectual Disability (QIDP)
Ability to be self-directed and resourceful; and participates in corporate quality improvement and problem-solving activities.
Ability to tolerate: Possible exposure to health risks (violence and infectious diseases, submit for annual TB Testing); and sometimes lifts, carries, or moves objects up to 25 lbs.
Experience with computer software - Word, Excel, and other electronic medical records
A valid Driver's License and good driving record


What you would be doing---


Providing wraparound facilitation services to include psychosocial assessments, treatment planning, coordination between providers, facilitation of family team meetings, and collaboration between community partners.
Operating under the guidance and clinical supervision of a master's level behavioral health clinician
Keeping confidentiality and privacy within standards
Actively supporting culturally competent, recovery-based practices, person-centered planning as a shared decision-making process with the individual, who defines his/her own life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture to aid individuals in their recovery process
Using a trauma-informed approach to service delivery emphasizes physical and emotional safety, choice, empowerment, and trustworthiness for customers
Maintaining record compliance within agency requirements and standards utilizing electronic / hard copy record formats
Receiving regularly scheduled clinical supervision and training in the core elements of case management and supports coordination

Professional office environment that generates noise from computer equipment/peripherals; or community setting/home setting with variable environmental factors., and at a Summit Pointe clinic locations (Calhoun County)
Typically day-shift, traditional business hours, with intermittent variable hours/shifts
