Job Description:
Primary Purpose This position will assist in the development,
implementation, and coordination of organization initiatives for
the improvement of clinical, operational, and administrative
services. This role supports a corporate wide quality improvement
program through establishing processes to understand and monitor
outcomes and collaborates with internal and external customers
across the organization to drive improvement. This position is
responsible for day-to-day quality and participant satisfaction
issues and facilitates improvement through ongoing analysis of data
to identify trends, significant results, and opportunities.
Supports regulatory compliance. Duties and Responsibilities
Interprets and communicates care management and quality results and
uses this information to influence process improvement. Exhibits
creative and effective problem solving and initiative in managing
change. Establishes and maintains positive working relationships
and networks effectively across all Senior Care Partners P.A.C.E.
departments to ensure a collaborative culture of quality throughout
the organization. Engages teams in the development and adoption
standard processes. Contributes to a learning culture of
improvement. Leads quality improvement projects using data to
define and measure problems and results. Conducts internal quality
auditing and monitoring efforts. Facilitates root cause analysis.
Collects and maintains the data and information required to be
reported to CMS and MDHHS pertaining to participant care activities
and outcomes. Contributes to the management of the participant
satisfaction program by ensuring proper resolution of grievances
and facilitation of the Participant Advisory Council. Orients new
staff and contracted providers on the quality improvement
principles and requirements. Supervisory Responsibilities This
position has no supervisory responsibilities. Qualifications
Bachelor---s Degree in nursing, statistics, organizational
psychology, industrial engineering, healthcare administration,
information systems. RN, LPN, or MA preferred. Process improvement
and Project Management experience required. Six Sigma, Lean or like
training and experience preferred. Strong interpersonal skills:
prior experience with team facilitation required. Demonstrated
ability in computer usage and software applications with a solid
knowledge in the use of Excel required additionally experience with
analytics software preferred. Minimum one year of experience
successfully leading employees completing quality related projects
or equivalent experience. Collaborative and supportive personality
that can inspire change in others. Minimum of one year of
experience working with the frail, elderly, or long-term care
population. Specific Requirements Immunizations and vaccines must
be current or medical documentation must be provided from a medical
provider with reason (s) to request waiver. (Hepatitis B, TB, MMR,
Varicella, TDaP, Flu, COVID-19) Must be able to read, write, speak,
and understand the English language and follow written and oral
instructions. Must demonstrate necessary skills and knowledge as
outlined in the position specific competency requirements. Must be
willing to seek out new methods and principles and willing to
incorporate them into existing nursing and/or medical practices.
Must care, support, and collaborate with everyone involved in
participant---s family using the patient-family centered care
approach. Must possess the ability to make independent decisions
when circumstances warrant such action. Must possess the ability to
communicate tactfully with personnel, participants, family members,
visitors, government agencies/personnel, and the public. Must have
proven leadership experience and skills with the ability to get
along harmoniously with others in a team environment. Must possess
the ability to plan, organize, develop, implement, and interpret
Senior Care Partners P.A.C.E. goals, objectives, policies, and
procedures, etc., that are necessary for providing quality care.
Must have patience, tact, a cheerful disposition, and enthusiasm,
as well as the willingness to handle difficult participants. Must
be knowledgeable of computers, data entry, output, system
applications, etc. Work Environment Moves intermittently during
working hours. Has a visible presence while working in the center
and is subject to frequent interruptions. Is subject to emotionally
upset participants, family members, and visitors, etc. Is subject
to callback during emergency conditions (e.g., severe weather,
evacuation, post-disaster, etc.). Works beyond normal working hours
when necessary. Is subject to pressure from multiple/emergency
calls. Physical Demands (With or without accommodations) Must be
able to move intermittently throughout the workday. Must be able to
read, write and speak the English language in an understandable
manner. Must be able to cope with the mental and emotional stress
of the position. Must function independently, have flexibility,
personal integrity, and the ability to work effectively with oher
personnel. Must have a pleasant speaking voice. Must be able to
push, pull, move, sit or stand for long periods of time. Lift up to
20 pounds as needed. May be necessary to assist in the evacuation
of participants during an emergency situation.
