Job Description:

The Nursing Assistant Pool will be assigned to patient care units depending upon the daily need. Assignments will be made by the Nurse Manager, designee, or Nurse Officer of the Day (NOD). The Nursing Assistant Pool are hired first as float staff. The Nursing Assistant Pool provides direct care and supervision to inpatients, as well as, outpatients. [ "The incumbent will be assigned to patients/residents requiring nursing care in all areas of patient care at this facility, such as medical, psychiatry, Ambulatory Care, Extended Care, including 1:1 constant observation assignments. Attends to patient/resident activities of daily living in assisting the patient/resident in obtaining maximum independence and self-care. Performs nursing procedures such as transferring and ambulation. Performs and documents the performance of nursing procedures (e.g. active and/or passive range of motion exercises (ROM): ambulation: application of splints, braces, and prosthetic devices; restorative dinning techniques; cognitive enhancement exercises; preventative skin care; bed mobility exercises; self-grooming and dressing techniques; toileting programs to promote continence; fall prevention: and additional programs) designed to assist and motivate the resident to achieve maximum independence in activities of daily living. Is alert to the potential development of contractures and pressure problems, reports these occurrences, and participates in identifying, implementing and documenting preventative interventions. Maintains the gravity drainage system once the ballooned Foley catheter has been inserted: and observes for patency, reporting catheter blockage or poor urinary output: provides ongoing catheter care (Foley/suprapubic) according to the procedures/standards. Takes emergency measures: calls for the nurse and/or physician and assists with resuscitation procedures in cardiac and/or respiratory arrest. Initiates CPR when necessary. Takes emergency measures by activating the psychiatric/medical emergencies as per nursing service guidelines. While off station calls 911 for emergencies. Promptly reports changes in the patient's/resident's behavior and or physical condition such as hemorrhaging, respiratory distress and the nursing intervention performed in the \"Progress Notes\" which are part of the patient's/resident's clinical record. Orients patient/resident to unit: observes and records patient's/resident's physical and emotional condition, care given, and patient's/residents' response in progress notes or flow sheets as required by documentation policies and/or guidelines. Is able to access the VA computer system and does this on a daily basis in order to receive information (i.e., electronic mail) and to document care given in appropriate databases and in the computerized progress notes. Completes assignments as posted on the daily assignment sheet: reports on unfinished tasks throughout the day: uses appropriate channels of communication (chain of command). Assignments to include maintaining safe and clean environment, and in the performance of additional duties as assigned. Plans, organizes and implements his/her nursing care assignment sheet for a group of patient's/residents; participates in discharge planning.\nRecognizes and protects patient's/resident's right to safety, privacy and self-determination. Applies supportive and restraint devices, documenting the care of these devices as required by policy or guideline. Provides nursing care to patient's/resident's requiring 1:1 observation. Provides post mortem care as per Nursing Guideline 118-5155. Under the direction of a RN, implements various devices to decrease falls and injuries. Contributes to the teaching of patient and family in rehabilitation techniques such as transferring techniques and ADL needs. Participates in orientation of new employees and assists less experienced employees to learn/perform ward activities. Assists with recreational and socialization activities and/or ward programs for patients/residents under the direction of a RN or trained leaders. In addition to the above, performs all duties outlined in the Nursing Assistant's GS-4 performance standards. Adheres to Nursing Service Guidelines and Medical Center Memorandums. Completes mandatory reviews and required in-service education in a timely manner. Adheres to and promotes the Medical Center's Quality Management, Infection Control, and safety programs. Reports safety hazards such as smoking risks and equipment in need of repair. Reports patient abuse. Utilizes principals of body mechanics in resident lifting procedures, equipment handling, and patient/resident care related activities. Uses patient/resident electronic or mechanical lift devices whenever possible. Obtains additional personnel when lifting, moving, or transferring. Is proficient in operation of various transportation equipment to include wheelchairs, power scooters, stretchers, and lifts both electronic and manual. When escorting patients/residents the employee is responsible for continual observation. The employee will report and record any unusual incidents to the patient's/resident's Charge Nurse and/or Nurse Manager or designee.
