a qualified Hospice RN in Fort Wayne, Indiana, 46825! Job Title:

Experience: Minimum of one (1) year experience as an RN in acute
care hospital, oncology, long-term care, hospice, or home health.
School internships and clinical hours may be considered in
individual cases. License: Current license to practice nursing in
the state of employment Certifications: BLS (American Heart
Association), current automobile insurance, and valid driver’s
license Must-Have: - Knowledge of terminally ill patients and their
families - Understanding of hospice care principles - Excellent
patient assessment skills - Good oral and written communication
skills - Knowledge of regulatory guidelines - Ability to operate
nursing assessment tools and basic office equipment -
Transportation for work Description: The Registered Nurse (RN) will
provide direct patient care services to hospice patients and
supportive care to their families. The RN will manage an
interdisciplinary team comprising Licensed Practical/Vocational
Nurses, Certified Nursing Assistants, and support staff to enhance
the quality of hospice care. Case management of all assigned
patient clinical records will be required. The RN ensures that the
highest quality of care is provided and documented in accordance
with the Plan of Care. Initial and ongoing assessments of the
patient will be performed. The RN will help develop and implement
the Hospice Plan of Care as prescribed by the physician. The RN
will work closely with the Interdisciplinary Group to provide
comprehensive physical, psychosocial, educational, emotional, and
spiritual services, including bereavement support. Any change in
the patient's condition must be reported to both the Attending and
Hospice Physician. An accurate and updated medical record must be
maintained, including all interdisciplinary notes and assessments.
Practicing infection control measures and standard precautions is
essential. The RN will monitor the scheduling of all staff and may
participate in the agency on-call schedule. Promotion of the
company’s core values is expected. Participation in the Quality
Assessment and Performance Improvement Program is required. The RN
must complete all required compliance training annually and
participate in staff meetings. A high level of knowledge pertaining
to pain and symptom management in the dying process must be
maintained. Routine supervisory visits with all team members will
be conducted at least monthly. An ongoing review and follow-up of
assigned patient clinical records are necessary to assure quality
services and compliance with policies. Maintaining close contact
with the patient’s family and significant others is essential for
providing information and support. The RN will act as the primary
liaison between the physician, patient/family, and hospice team.
Education of the patient, family, or other caregivers regarding
needs identified on the Plan of Care is the RN's responsibility. 5
