Job Description Corewell Health West Medical Group is seeking a
BC/BE Internist or Family Medicine Physician with either a
Geriatrics Fellowship or demonstrated experience in geriatrics to
join our Home Based Care Program specializing in the care of our
elderly and medically frail population at Corewell Health West in
Grand Rapids, Michigan. Enjoy a high level of clinical, clerical,
and administrative support while developing your Geriatric
practice. A team-based, in-home primary care program which provides
physician/advanced practice professionals, RN care managers, social
workers, and medical assistants to 830 frail, elderly patients in a
10-county area in West Michigan. This value- based model of care is
funded via Corewell Health’s insurance plan, Priority Health.

Qualifications: BE/BC in Internal Medicine or Family Medicine
Geriatrics Fellowship Preferred License - Michigan MD/DO
