Summary:

THIS INDIVIDUAL IS RESPONSIBLE FOR COORDINATING ALL SERVICES THAT ARE RECEIVED THROUGH THE INTAKE DEPARTMENT. REFERS TO OTHER COMMUNITY RESOURCES AS NECESSARY.



DEMONSTRATES EXCELLENT CUSTOMER SERVICE PERFORMANCE IN THAT HIS/HER ATTITUDE AND ACTIONS ARE AT ALL TIMES CONSISTENT WITH THE STANDARDS CONTAINED IN THE VISION, MISSION AND VALUES OF COVENANT AND THE COMMITMENT TO EXTRAORDINARY CARE FOR EVERY GENERATION.



Responsibilities:

DEMONSTRATES EXCELLENT CUSTOMER SERVICE SKILLS WITH ALL INTERNAL AND EXTERNAL CUSTOMERS.

REPRESENTS THE VNA IN A POSITIVE, COOPERATIVE MANNER WITH EXCELLENT PHONE ETIQUETTE AND EXCELLENT COMMUNICATION SKILLS.

MAINTAINS CONFIDENTIALITY WITH ALL INFORMATION IN THE APPROPRIATE MANNER.

CONTRIBUTES TO ORGANIZATION SUCCESS TARGETS FOR PATIENT SATISFACTION AND NET OPERATING MARGIN.

PROVIDES EDUCATION TO ALL CUSTOMERS ON HOME CARE REQUIREMENTS.

TRIAGE CALLS REGARDING PATIENT S HEALTH CARE NEEDS AND COORDINATES WITH OTHER DEPARTMENTS AND/OR OTHER ORGANIZATIONS AS NEEDED.

ACTS AS LIAISON BETWEEN PHYSICIAN AND HOMECARE STAFF (VERBAL ORDERS; FOLLOW UP CALLS, LABS, ETC).

INTERPRETS AND SECURES INSURANCE REQUIREMENTS FOR SERVICES PROVIDED. PROFICIENT WITH HOSPITAL AND HOMECARE COMPUTER PROGRAMS.

ABILITY TO PROVIDE GOOD CUSTOMER SERVICE WHILE MULTI-TASKING.

COMPLETE APPROPRIATE/NECESSARY DOCUMENTATION IN A TIMELY FASHION.

COMMUNICATES IDENTIFIED ISSUES/VISIT REASON WITH THE CASE MANAGER.

UPDATES POC AS NECESSARY.

RESPONSIBLE FOR SPECIAL PROJECTS AND OTHER DUTIES AS ASSIGNED.

SUPPORTS ORGANIZATIONAL POLICIES.

NOTIFIES APPROPRIATE LEADERSHIP OF PATIENT ISSUES OR CONCERNS AND FOLLOWS THROUGH AS APPROPRIATE.

MAINTAINS RECORDS IN ACCORDANCE WITH AGENCY REQUIREMENTS.

INFORMAL CHART REVIEW.

CONTACT APPROPRIATE LEADERSHIP IN REGARDS TO ACCEPTING REFERRALS.

ABILITY TO PERFORM HOME VISITS AS NEEDED.

EXHIBITS EXCELLENT PROFESSIONAL STANDARDS AND UTILIZES CRITICAL THINKING AND PROBLEM SOLVING SKILLS TO MEET THE NEEDS OF ALL VNA CUSTOMERS.

EDUCATION/EXPERIENCE:

REGISTERED AS A LICENSED PRATICAL NURSE IN THE STATE OF MICHIGAN.

FIVE YEARS OF NURSING EXPERIENCE AND TWO YEARS OF HOME CARE EXPERIENCE.



KNOWLEDGE/SKILLS/ABILITIES:

KNOWLEDGE IN CURRENT NURSING PRACTICES.

ABILITY TO RELATE POSITIVELY TO PATIENTS, FAMILIES, AND CO-WORKERS.

ABILITY TO READ, WRITE, SPEAK, AND COMPREHEND ENGLISH.

SECOND LANGUAGE DESIRED BUT NOT REQUIRED.

KNOWLEDGE OF BASIC COMPUTER PROGRAMS.

ABLE TO OPERATE WITH MINIMAL SUPERVISION.

KNOWLEDGE REGARDING AVAILABILITY OF COMMUNITY RESOURCES.

VALID MICHIGAN DRIVER S LICENSE.

VALID MICHIGAN NURSING LICENSE.

ABILITY TO TYPE 40 WPM.

NEAR VISUAL ACUITY AND FUNCTIONAL HEARING REQUIRED.
